Registration Checklist
- Proper identification of each child (Birth certificate or letter of live birth)
- Shot record prior to enrolling.
- A physical is due prior to or within the first 30 days of enrollment.
- Two emergency contacts phone & address is required.
- Signed payment contract
- Complete registration form
- Emergency medical consent form
- A Parent Handbook is given to each family upon enrollment and serves as part of the parent contract.
